About the role

Self Regional Healthcare is a well-respected organization committed to continually advancing care for the communities we serve. The Licensed Professional Counselor provides therapeutic counseling to patients and family members/families, completes needs assessments, develops intervention plans, conducts group therapy sessions, and makes appropriate referrals to community resources. The role involves telephone triage for patients in crisis, managing IN Baskets messages, and providing immediate intervention when necessary. Proficiency in the EPIC electronic medical record system is required for documentation.

Responsibilities

  • Provide therapeutic counseling to patients and family members/families.
  • Complete needs assessments based on clinical input from primary care providers, psychiatrists, and others familiar with the patient.
  • Develop plans for intervention, care, or assistance.
  • Conduct group therapy sessions as appropriate for the population.
  • Make appropriate referrals to other community resources when needed.
  • Contact patients and family members as needed and document these interactions.
  • Perform telephone triage for patients in crisis.
  • Answer all IN Baskets messages and provide immediate intervention when necessary.
  • Use the EPIC electronic medical record system for documentation of all patient visits and correspondence.

Requirements

  • Master's degree in mental health counseling or other related field.
  • Independent licensure (e.g., Licensed Professional Counselor or Licensed Independent Clinical Social Worker).

The role is based in the Greenwood Area, part of the Lakelands region in South Carolina.
