Licensed Practical/ Vocational Nurse

About the role

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN/ LVN) to join our healthcare team at Forest Hills Commons. Our team creates a welcoming environment where residents feel at home, supported, and valued.

Responsibilities

• Administer medications, treatments, and wound care as prescribed by physicians.
• Monitor and assess patient conditions and promptly report changes.
• Absorb and manage patient interactions.
• Provide compassionate care, ensuring patients' comfort and dignity.
• Maintain accurate patient records and documentation in accordance with facility policies.
• Collaborate with doctors, CNAs, and other healthcare team members to deliver comprehensive care.
• Educate patients and families on health management and discharge instructions.

Requirements

• Current LPN/LVN license in KY
• Strong clinical and patient management skills
• Excellent communication and interpersonal skills
• Ability to work in a fast-paced environment and handle challenging situations with grace
• Basic Life Support (BLS) certification required; Advanced Cardiac Life Support (ACLS) preferred
