Licensed Practical/Vocational Nurse

About the role

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment.

Responsibilities

• Outpatient/Primary Care:
  • Provides direct patient care to and assists with care coordination of Primary Care patients within the Patient Aligned Care Team (PACT).
  • Serves as a point of contact for patients and families regarding their care and treatment plans.
  • Participates in PACT performance measure activities such as non-complex 2-day post-discharge calls, clinical reminder compliance, and aspects of chronic disease management.
  • Assists with coordinating care of patients, including those seen through Clinical Video Tele-Health visits.
  • Alerts providers (physicians/nurse practitioners) of abnormal signs and symptoms and/or abnormal test results to expedite care.
  • Affirms patients have pre-appointment requirements completed prior to Primary Care Provider (PCP) appointments.
• Inpatient:
  • Delivers care to patients according to Patient Care Plan.
  • Understands age and population-specific characteristics that affect the health care needs of the Veteran.
  • Performs aspects of admission/discharge procedures as directed.
  • Captures patient data including observations of patient status (i.e., vital signs, intake/output, skin condition, mental status, etc.).
  • Accurately reports patient/client information.
  • Prompts recognition of subtle and significant changes in the patient in response to nursing care.
  • Recognizes urgent or emergent patient care situations and seeks assistance and effectively intervenes as directed.
  • Organizes/prioritizes routine and non-routine nursing care activities (ADLs, medication administration, wound care, EKG, Foley catheter insertion, bladder scan, etc.).
  • Provides care to patients/clients appropriate to their cognitive, physical, emotional, and chronological maturation.

Qualifications

• Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant.
• Current active full unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse in a State, Territory, or Commonwealth of the United States or the District of Columbia.
• May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Skills

• Knowledge and ability to prepare, administer, and document actions taken specific to commonly prescribed medications.
• Recognition and response to emergency measures and seeking assistance from RN or MD/DO.
• Observation and documentation of patient's response to medication administered and reporting any noted changes to RN or MD/DO.
• Effective communication and education to patients and/or family members in relation to common disease processes, medication, and prescribed treatment regimes.
• Ability to recognize and respond to breakage/malfunction or loss of equipment, safety hazards, and supply deficiencies.
• Ability to organize and deliver care considering emotional, socio-economic, and age-related factors.
• Knowledge and ability to recognize urgent or emergent patient care situations and initiate appropriate emergency interventions as directed.
• Recognition and understanding of human behavior and ability to work effectively with patients, family members, and other staff.
