About the role

Full-time night shift (11 p.m.–7 a.m.) Licensed Practical Nurse providing direct patient care to residents in a skilled-nursing and rehabilitation center under the direction of a professional nurse.

Responsibilities

• Assist in the implementation of individualized treatment plans for each assigned resident.
• Supervise Certified Nursing Assistants (CNAs) and participate in their evaluation.
• Oversee Falls Program and Pressure Sore Program.
• Provide nursing services and conduct/document thorough medical-status evaluations upon admission and throughout the resident’s course of treatment.
• Other duties as assigned.

Requirements

• Current, unencumbered, active state LPN license.
• Experience in skilled-nursing/rehabilitation facilities preferred.
• Compassionate, dedicated, and detail-oriented.
