Licensed Practical Nurse - Support Center
Position Summary
The Support Center LPN is a member of the interdisciplinary team who work together to meet the physical, medical, psychosocial, emotional, and spiritual needs of BCN’s patients and families facing serious illness. The Support Center LPN performs telehealth triage functions including but not limited to receiving calls; providing phone support; providing interventions utilizing BCN policies, procedures, and protocols; dispatching on-call nurses as needed. The Support Center LPN completes documentation utilizing the EMR, point of service.
Position Qualifications
- A graduate of an accredited practical nurse or vocational nursing program.
- Has at least two (2) years of nursing experience.
- Community Health/Hospice or long-term care experience preferred.
- Telehealth experience also preferred.
- Is currently licensed as an LPN through the State Board of Nursing.
- Has excellent assessment, nursing judgment, and communication skills.
- Able to cope with heightened family emotional stress, and tolerant of individual lifestyles.
- Complies with accepted professional standards and principles.
- Complies with strict adherence to organizational policies, procedures, and protocols.
- Basic computer skills are required. Advanced computer skills are preferred.
- Is self-directed with the ability to work with little supervision.
- Is flexible and cooperative in fulfilling role obligation and has the ability to work with an interdisciplinary team.
Additional Information
Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies. Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.
