Licensed Practical Nurse Supervisor - Personal Care - Shenango on the Green

About the role

The Licensed Practical Nurse (LPN) Supervisor at Shenango on the Green, New Wilmington, PA, is responsible for coordinating and supporting the individualized plan of care for community residents. This position supervises and provides leadership and direction to the team to ensure quality care and services are provided.

Responsibilities

• Coordinates and supports the individualized plan of care for community residents.
• Supervises and provides leadership and direction to community team members to ensure quality care and services are provided to residents.
• Works closely with residents and their families to understand their needs and preferences.
• Ensures compliance with all regulatory standards and policies related to nursing care.
• Collaborates with interdisciplinary teams to develop and implement care plans.
• Assesses resident needs and makes recommendations for adjustments to the care plan as needed.
• Trains and mentors new LPNs and other team members.
• Maintains accurate documentation of care provided and resident progress.

Requirements

• Hold a valid, current Practical Nurse License in good standing.
• Possess excellent communication and customer service skills.
• Ability to effectively interact with both residents and team members.
• Demonstrate excellent teamwork.
• Experience in Senior Living preferred.
• Experience in memory care (dementia or Alzheimer’s) preferred.
• Proven leadership experience preferred.

Pay

$25.16/hour - $39.02/hour; Depending on experience

Schedule

(5) 8-hour overnight Shifts from 10:30 pm to 6:30 am, or (2) 12 hr. shifts from 6:30 pm to 6:30 am and (2) 8-hour shifts from 10:30 pm to 6:30 am, with rotating weekends and holidays
