Licensed Practical Nurse Supervisor (3063)

Pay: $42.00-$44.00/hour

Schedule

• Week 1: Sunday 3pm-11pm, Monday 3pm-11pm, Tuesday 9am-5pm, Wednesday 9am-5pm, Thursday 9am-5pm
• Week 2: Monday 3pm-11pm, Tuesday 9am-5pm, Wednesday 9am-5pm, Thursday 3pm-11pm, Saturday 3pm-11pm
• Mandatory staff meeting every 3rd Thursday of the month 1pm-3pm

Responsibilities

• Provide high-quality nursing care and medication administration to individuals in accordance with physicians' orders and agency protocols
• Serve as a clinical resource and mentor to other LPNs and direct care staff within assigned programs
• Support the onboarding, orientation, and training of nursing staff, including oversight of clinical competencies
• Monitor the implementation of health care plans and treatment protocols
• Assist in the scheduling and coordination of nursing coverage across programs, ensuring consistent and adequate staffing
• Communicate medical issues promptly and effectively to RNs, Health Care Directors, physicians, and program leadership
• Conduct or assist in health assessments and ensure timely documentation in Electronic Health Records (EHR)
• Accompany individuals to medical appointments when necessary, and ensure follow-up care is completed and documented
• Monitor and maintain compliance with Medication Administration Program (MAP) standards and provide MAP training as required
• Participate in incident reporting, follow-up, and resolution in collaboration with clinical and administrative leadership
• Ensure health records and documentation are complete, accurate, and compliant with DDS and agency standards
• Promote a culture of respect, safety, and dignity in all interactions with individuals and staff
• Perform other nursing or administrative duties as assigned by the supervising RN or Health Care Director

Requirements

• Associate's degree required
• Standard driver’s license – class C or class D
• Licensed practical nurse (LPN) license required
• 1 – 3 years of related experience
• Possess a working knowledge of the medical needs of individuals with intellectual and developmental disabilities, with a minimum of two years of supervisory experience
• Must be able to drive wheelchair van when necessary (training will be provided)
