Licensed Practical Nurse – Skilled Nursing & Rehab

Role: Licensed Practical Nurse (LPN) Responsibilities: Provide person-centered, bedside care to residents in a skilled nursing and rehabilitation setting.Administer medications and treatments accurately and safely.Monitor, assess, and document residents' conditions and progress.Collaborate with RNs, physicians, and the interdisciplinary team to develop and support care plans.Educate residents and their families about health, treatments, and safety protocols.Promote safety, infection control, and quality improvement initiatives. Qualifications & Skills: Valid LPN license.Strong patient care skills with attention to detail.Excellent communication and teamwork abilities.Knowledge of infection control and safety standards.Commitment to dignity, respect, and compassionate care.
