Licensed Practical Nurse – Skilled Nursing & Rehab

Role: Licensed Practical Nurse (LPN) Responsibilities Provide person-centered, direct bedside care to residents in a skilled nursing and rehabilitation setting.Administer medications and treatments as prescribed.Monitor, assess, and document resident conditions accurately.Collaborate with RNs, physicians, and the interdisciplinary team to develop and support care plans.Educate residents and families on health, treatments, and safety protocols.Promote safety, infection control, and adherence to quality standards. Qualifications & Skills Licensed Practical Nurse (LPN) licensure.Strong communication and teamwork skills.Ability to provide compassionate, person-centered care.Knowledge of medication administration, documentation, and safety procedures.
