About the role

Responsibilities
- Monitor patient progress, report health changes, and perform detailed observations within LPN scope.
- Deliver personalized nursing care, continuously evaluating and adjusting care plans to meet evolving needs.
- Coordinate care by communicating with interdisciplinary teams and other providers to ensure effective service delivery.
- Travel to patients’ homes or facilities, managing caseloads efficiently with strong time management.
- Provide education, support, and resources to patients and families, including end-of-life care guidance.
- Utilize technology such as tablets and EMRs to maintain accurate patient records and coordinate care.
Qualifications
- Current LPN license and registration in New York State.
- High School Diploma or LPN Program completion.
- Work experience as a new graduate or bilingual skills may be preferred.
- Valid driver’s license may be required based on operational needs.
