Our Home Care LPNs deliver patient-centered care in community and home settings, helping patients heal and age comfortably. They provide individualized, clinical care under RN supervision, ensuring positive health outcomes. Responsibilities

• Monitor patient progress, report health changes, and perform detailed observations within LPN scope.
• Deliver personalized nursing care, continuously evaluating and adjusting care plans to meet evolving needs.
• Coordinate care by communicating with interdisciplinary teams and other providers to ensure effective service delivery.
• Travel to patients’ homes or facilities, managing caseloads efficiently with strong time management.
• Provide education, support, and resources to patients and families, including end-of-life care guidance.
• Utilize technology such as tablets and EMRs to maintain accurate patient records and coordinate care.

Qualifications

• Current LPN license and registration in New York State.
• High School Diploma or LPN Program completion.
• Work experience as a new graduate or bilingual skills may be preferred.
• Valid driver’s license may be required based on operational needs.
