Licensed Practical Nurse (Part-time)

About the role

The Clinical Health Advocate (Licensed Practical Nurse) is an integral part of our care team, owning patient relationships and driving a collaborative care plan that impacts patients' lives meaningfully. This is a part-time (up to 24 hours/week) hybrid role combining clinical and administrative responsibilities.

This role involves:

• At patient visits, obtaining and documenting vital signs, performing venipuncture and capillary blood collection, collecting and processing specimens, administering immunizations, assisting with minor procedures, and educating patients on treatment plans and follow-up care.
• Relaying environmental factors impacting patients' wellbeing, conducting physical and social screenings, managing exam room and mobile unit cleanliness, coordinating insurance submissions, managing inventory, and communicating with patients via text, video, or phone.
• Managing patient chart management, ensuring thoroughness of care, running-the-list, and coordinating care plans; submitting Pop Health documents to maximize compliance with payer contracts as needed.

Qualifications

• At least 2 years clinical experience as a Licensed Practical Nurse
• Experience with phlebotomy
• Experience with injections (vaccines) preferred
• Reliable access to personal transportation
• Strong people skills and the ability to connect with anyone
• Creative problem-solving skills
• Strong communication skills, especially the ability to translate complex ideas into simple language
• A "hospitality mindset" and track record of creating memorable experiences for others
• Ability to manage multiple priorities at once

Pay

$27/hour
