Licensed Practical Nurse (Part-Time, 1st and 3rd Shift)

• Deliver individualized care per the care plan, including medication administration, wound care, and support with personal needs.
• Monitor patient condition, assess changes, and document accurately, reporting any concerns promptly.
• Support clients with specialized needs such as trach or ventilator care, and assist with procedures as trained.
• Educate and empower clients and families on routines, equipment, and symptom management.
• Collaborate with the care team to ensure continuity, effective communication, and quality outcomes.

Qualifications & Skills

• Graduate of an accredited practical nursing program with an active LPN/LVN license.
• Current CPR/BLS/First Aid certification.
• Preferably with home care or private duty experience; trach/ventilator experience is a plus.
• Reliable transportation, ability to observe and assess patient status, and physical capability to lift and reposition patients.

Work Environment & Physical Demands

Primarily in clients' homes, involving frequent exposure to infectious materials and requiring safety precautions. Ability to perform medium physical activity, including lifting up to 50 lbs., with consistent shift-based coverage.
