Licensed Practical Nurse - 3rd Shift

About the Role

A Private Duty Nurse (PDN) Licensed Practical Nurse (LPN) provides one-on-one in-home skilled nursing care, supporting clients with personalized, continuous care to promote safety, comfort, and independence at home.

Responsibilities

• Administer medications, treatments, and support with personal care per care plan.
• Assess, monitor, and document patient conditions, reporting changes promptly.
• Provide skilled procedures such as wound care, trach care, ventilator support, and enteral feeds.
• Educate clients and families on routines, equipment, and symptom management.
• Coordinate care with the team, ensure continuity, and communicate effectively during shift handoffs.

Requirements

• Graduate of an accredited practical nursing program with an active LPN/LVN license.
• Current CPR/BLS/First Aid certification.
• One year of clinical experience preferred; home care experience a plus.
• Reliable transportation, with proof of driver’s license and auto insurance if needed.
• Trach and/or ventilator experience preferred; training may be provided.
• Ability to perform physical activities including lifting up to 50 lbs and frequent repositioning.

Work Environment

Care is primarily provided in clients' homes, with exposure to infectious materials requiring safety precautions. The role involves medium physical activity, including lifting and repositioning patients, and may include community-based settings.
