Licensed Practical Nurse - Outpatient Adult South

Essential Job Functions

• Administers and oversees self-administered and prescribed medications per established schedules as ordered.
• Maintains accurate records reflecting vital signs, physical health information, and treatment results.
• Administers injectable medications, ensuring accurate dosage and timely delivery, and educates clients on managing side effects.
• Completes and maintains accurate documentation of care, medications, and other client-related services.
• Maintains client medical records and charts for admissions and discharges.
• Transcribes medical provider orders and obtains requested medical information.
• Maintains accurate inventory counts of medicines and medical supplies and ensures timely replacement orders.
• Schedules outside medical appointments for clients, as ordered by medical providers.
• Maintains medical equipment and ensures compliance with CLIA standards.
• Participates on a team with MDs/APRNs/RNs to manage medication management clinics.

Education and Training

• Up to eighteen months of education or training beyond the high school level in nursing or related field of study.
• Licenses for Practical Nursing in the Commonwealth of Kentucky.
• Graduation from an accredited program.

Experience

• Minimum 6-12 months nursing experience.

Physical Demands

• Occasional exposure to threat of bodily harm from clients and their families.
• Routine but limited exposure to blood, body fluids, or tissue.
• Reliable transportation for frequent travel between work sites.
