About the role

We are seeking compassionate and committed Licensed Practical Nurses (LPNs) to join our clinical team at Bluestone Health & Rehabilitation /Bluestone Senior Living in Bluefield, West Virginia. Our facility is a top-rated Long-Term Care and Skilled Nursing Facility dedicated to providing exceptional care and dignity to our residents.

Responsibilities

  • Provide nursing care to ensure resident needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines
  • Administer medications and treatments with precision and compassion
  • Collaborate with RNs, CNAs, and interdisciplinary teams
  • Support residents’ physical, emotional, and psychological well-being
  • Absorb and assist with resident comfort
  • Collect samples and monitor catheters
  • Provide wound care
  • Keep resident patient information confidential, follow all policies and procedures, and adhere to federal, state, local requirements, and professional standards
  • Collect, track, maintain, and review patient medical history, charts, and physical condition, including heart rate, blood pressure, temperature, and respiration
  • Help coach staff as necessary

Requirements

  • Current LPN license in [State]
  • CPR/BLS certification (or willing to obtain)
  • Previous LTC or SNF experience preferred, but new grads welcome!
  • Positive attitude, reliability, and strong clinical judgment
