About the role

The Licensed Practical Nurse (LPN) at Manchester Commons in Erie, PA, coordinates and supports individualized service plans for community residents. This role supervises and provides leadership to the community team to ensure high-quality care and services.

Responsibilities

• Coordinates and supports individualized service plans for community residents.
• Supervises and provides leadership to the community team to ensure quality of care and services.
• Works closely with interdisciplinary teams to develop and implement care plans.
• Assists with medication management and administration.
• Provides direct patient care, including bathing, grooming, feeding, and mobility assistance.
• Documents resident care activities accurately and timely.
• Ensures compliance with regulatory standards and policies.

Requirements

• Current, valid Practical Nurse License in good standing from the state of Pennsylvania.
• Experience as a licensed nurse preferred.
• Experience in Personal Care or Assisted Living preferred.
• Knowledge of DHS regulations preferred.
• Dementia care experience preferred.
• Maintains CPR certification.

Pay

Premium Program Rate: $33.00/hour + $3,000 Sign-On Bonus
