Our interdisciplinary team works together to help residents recover, regain independence, and enjoy the highest possible quality of life. 🩺 Position Overview As a Licensed Practical Nurse (LPN), you will provide direct nursing care to residents while collaborating with Registered Nurses, physicians, therapists, and other healthcare professionals. Your clinical expertise and compassionate approach help ensure residents receive exceptional care every shift. ✅ Key Responsibilities Administer medications and treatments as prescribedMonitor, assess, and document residents' health status, including vital signsReport changes in condition to the supervising RN or physicianAssist residents with activities of daily living (ADLs) as neededCollaborate with the rehabilitation team to support recovery goalsMaintain accurate documentation in accordance with facility policies and state/federal regulationsProvide education and emotional support to residents and familiesFollow infection control and safety protocols to maintain a safe environment 🎓 Qualifications Current and valid Licensed Practical Nurse (LPN) licenseStrong clinical skills, including medication administration and wound careExcellent communication and interpersonal skillsAbility to work effectively in a team-oriented environmentCompassionate, patient-focused approach to careBLS/CPR certification required 🌟 Why Join Our PRN Team?
