Responsibilities
- Affirms and evaluates patient status by adhering to the plan of care, regularly reassessing patient and family/caregiver needs, and modifying the plan of care as needed based on patient status and needs in collaboration with the RN Case Manager.
- Conducts a focused physical assessment considering the physical, emotional, and spiritual aspects that may impact health, well-being, and quality of life.
- Attends and offers support during the time of death (in accordance with physician's order) and in compliance with local regulations.
- Maintains open communication with the attending physician and RN Case Manager regarding patient needs and updates these communications as conditions change; obtains/receives physicians' orders as necessary.
- Maintains accurate and timely documentation of clinical records in electronic format to ensure compliance with regulatory standards, reflecting changes as they occur.
Requirements
- Graduate of an Accredited Practical Nurse or Vocational Nurse Program.
- Holds current unencumbered license in the state of practice as a Licensed Practical Nurse.
- Minimum three (3) years' experience as LPN in Community Health, Medical surgical/Acute care setting required. Hospice/home health experience preferred but not required.
- Must be computer proficient in typing and various programs, including familiarity with Electronic Medical Records (EMR).
- Possesses and maintains current CPR certification if required by state.
Qualifications
- Graduate of an Accredited Practical Nurse or Vocational Nurse Program.
- Holds current unencumbered license in the state of practice as a Licensed Practical Nurse.

Pay
$39.00 - $40.00 per hour (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
