Licensed Practical Nurse (LPN) - Private Duty Home Health

About the role

Provide personalized, high-quality in-home care for clients in their homes.

Responsibilities

• Administer medications, treatments, and therapies as prescribed.
• Monitor and report clients' condition changes to supervising healthcare professionals.
• Assist with activities of daily living such as bathing, dressing, and grooming.
• Educate clients and families on health care and maintenance.
• Maintain accurate documentation and follow agency policies and regulatory guidelines.

Requirements

• Valid LPN license in the state.
• At least 1 year of experience, preferably in home health care.
• Current CPR certification and Level 2 Background Screening.
• Strong clinical, critical thinking, communication, and interpersonal skills.
• Compassionate, patient-focused, and reliable.
