AWESOME PERKS: Only on call for triage of emergencies 1 week per monthMileage & Cell Phone ReimbursementFlexible Hours –5 day work week (40 hours, Monday thru Friday) NO Weekends Typically 8am-5pmLow stress, relaxed environmentCase management role with semi-annual physical assessments.High morale, family like, team oriented environmentPaid trainingWhat You Get To Do: Ensure that each individual receives quality, individualized health care management, resulting in their optimal health Conducting routine site visits, checking vitals, doing assessments, counseling of medication administration, and documenting progress Act as a health/medical resource for staff, training staff in the medical arenaConduct audits and reviews of medicationsEnsure that the company is in compliance with state requirements and organizational policies regarding medical care and records Schedule: Full-time Monday-Friday with occasional travel required; flexible hoursParticipate in an on-call rotation for triage of medical concerns for 1 week per month Qualifications What Makes You A Great Fit: Current Licensed Practical Nurse (LPN) license in the State of Indiana required1-2 years experience working with individuals with disabilities or other long-term care is preferredMust be skilled, flexible, self-motivated, dependable, and able to work cooperatively with staff, interdisciplinary team members, and outside agenciesStrong verbal/written communication skillsAbility to work independently and efficientlyOrganizational and computer skills Acceptable background check & driving record check Additional Information We believe in supporting not only those we serve but also each other, ensuring a collaborative and supportive work environment for all.
