About the Role

Ten Broeck Center for Rehabilitation and Nursing is seeking a Full-Time Licensed Practical Nurse (LPN) to join our team at 1 Commons Drive, Lake Katrine, NY 12449. We are a 258-bed facility recognized for our outstanding delivery of subacute and long-term care, providing a calm, friendly, and healthy environment for our residents and employees.

Responsibilities

• Admissions, discharges, and transfers of residents
• Medication requisitions with pharmacy and physicians
• Medication administration
• Following infection control, emergencies, and other center policies
• Collaborating with the resident’s interdisciplinary team to provide the best outcomes
• Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy
• Working under direct supervision in accordance with state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment
• Delivering nursing care to patients/residents requiring long-term or rehabilitative care
• Collecting patient/resident data, making observations, and reporting pertinent information related to patient/resident care
• Implementing the patient/resident plan of care and evaluating the patient/resident response according to state-specific regulations
• Directing and supervising care given by other nursing personnel in selected situations, in accordance with state-specific regulations
• Maintaining knowledge of necessary documentation requirements
• Maintaining knowledge of equipment set-up, maintenance, and use (e.g., monitors, infusion devices, drain devices)
• Maintaining confidentiality and patient/resident rights regarding all patient/resident/personnel information

Qualifications

• LPN (Licensed Practical Nurse) licensed by the State
• CPR Certification required
• Two years of prior nursing experience preferred
