Licensed Practical Nurse (LPN) - Mankato, MN

Key Responsibilities
- Provide compassionate nursing care in community and residential settings.
- Deliver care according to LPN licensure, policies, and individualized service plans.
- Train, coach, and support direct care staff to ensure safe, high-quality care.
- Maintain accurate medical records and ensure regulatory compliance.
- Help staff recognize illness symptoms and provide appropriate in-home care.
- Complete required health documentation and incident reporting.
- Assist with onboarding and ongoing clinical training for staff.
- Monitor medication administration, track errors, and support corrective actions.
- Participate in scheduled on-call rotation.
- Communicate with physicians, pharmacies, and other healthcare providers.
- Provide hands-on training in medication administration, CPR, and health-related topics.
- When providing direct care, assist individuals with ADLs, community activities, household tasks, and medications.
- Maintain confidentiality and follow all HIPAA, safety, and infection control protocols.
Requirements
- Graduate of an accredited school of nursing; holds a current Minnesota Nursing License.
- Valid Driver’s License and pass a driving record check conducted by our insurance carrier unless otherwise noted.
- Excellent verbal and written communication skills, including the ability to effectively communicate with individuals and coworkers.
- Self-motivated and exhibits superior interpersonal communication skills.
- Positive attitude towards the job.
- Basic knowledge of technology programs, such as Microsoft Office products, and ability to use or learn technology for daily charting.
- Team player with the ability to work independently and carry out job duties as assigned.
- Exceptional documentation skills and knowledge of systems, routines, and programming.
- Pass the MN Department of Human Services Background Study.

Pay
- $27.00 to $30.00 per hour depending on experience.
Schedule
- Monday through Friday, with normal business hours, with flexibility to adjust hours earlier or later based on needs of the individuals or scheduled appointments.
