About the Role
Responsible for providing professional patient care to residents, supervising Certified Nursing Assistants (CNAs), and ensuring accurate documentation in a long-term care setting.
Responsibilities
- Administer medications accurately and document accordingly.
- Perform treatments and monitor patient progress per physician orders.
- Ensure comprehensive resident care documentation using electronic records.
- Lead CNA team in delivering personalized care aligned with care plans and verify documentation accuracy.
- Communicate effectively with staff, physicians, and supervisors through reports and order transcriptions.
- Monitor residents' conditions and promptly report changes.
- Complete medication record reviews and Minimum Data Set (MDS) documentation timely.
- Assist with direct Activities of Daily Living (ADL) care when needed.
Requirements
- Graduate of an accredited School of Practical Nursing.
- Current New York State licensure as a Licensed Practical Nurse (LPN).
- Basic Life Support (BLS) certification.
- Preferred: 1 year of experience in a healthcare setting.
- Physical ability to perform medium work involving walking, standing, and lifting.
