Licensed Practical Nurse (LPN) - Long Term Care (Part-Time, Night Shift)

About the Role

Responsible for providing professional patient care to residents, supervising Certified Nursing Assistants (CNAs), and ensuring accurate documentation in a long-term care setting.

Responsibilities

• Administer medications accurately and document accordingly.
• Perform treatments and monitor patient progress per physician orders.
• Ensure comprehensive resident care documentation using electronic records.
• Lead CNA team in delivering personalized care aligned with care plans and verify documentation accuracy.
• Communicate effectively with staff, physicians, and supervisors through reports and order transcriptions.
• Monitor residents' conditions and promptly report changes.
• Complete medication record reviews and Minimum Data Set (MDS) documentation timely.
• Assist with direct Activities of Daily Living (ADL) care when needed.

Requirements

• Graduate of an accredited School of Practical Nursing.
• Current New York State licensure as a Licensed Practical Nurse (LPN).
• Basic Life Support (BLS) certification.
• Preferred: 1 year of experience in a healthcare setting.
• Physical ability to perform medium work involving walking, standing, and lifting.
