Licensed Practical Nurse (LPN) Long Term Care (Full-Time, Evenings)

Unity Living Center is a 120-bed, state-of-the-art skilled nursing facility located on two recently renovated floors within the Rochester Regional Health St. Mary’s campus. Our team provides specialized, patient-centered care for individuals with complex medical needs, dementia, and behavioral challenges, creating an environment where nurses can grow their clinical expertise while making a meaningful impact.

About the role

As a Licensed Practical Nurse, you will provide professional patient care to long-term and post-acute care residents. You will direct and monitor the care provided by CNAs and consistently ensure proper documentation and recording is taking place. Your ability to lead is crucial in both the success of the CNAs and the care of the patients.

Responsibilities

• Responsible for timely, accurate medication administration of assigned team and concurrent required documentation.
• Performs treatments and documents progress as per physician orders and facility protocol.
• Assures concise, pertinent, and complete resident care documentation using computerized medical record process.
• Assures the initiation of appropriate care pathway tools.
• Directs CNAs assigned to the team in the timely completion of quality individualized personal care in compliance with resident's individual care plans.
• Ensures CNA documentation is accurate and complete.
• Responsible for receiving/giving relevant reports, both written and verbal, from/to appropriate staff as per departmental protocols.
• Contacts physicians, transcribes orders, and follows up with appropriate action to institute orders in a timely manner.
• Assures proper monitoring of residents and relevant condition status are reported to charge nurse/supervisor and/or physicians in a timely manner.
• Completes pre-reviews of medication administration records as per facility protocols.
• Completes required MDS documentation and all related observations in a timely manner.
• Supports CNA team through assisting in direct ADL care of residents.

Requirements

• Graduate from an accredited School of Practical Nursing.
• Current LPN licensure in the State of New York.
• Basic Life Support (BLS) Certification from the American Heart Association (AHA).

Preferred Qualifications

• 1 year of experience.

Skills

• Build specialized clinical expertise with hands-on experience in short-term rehabilitation, IV therapy, peritoneal dialysis, hemodialysis, ventilator care, pain management, and chronic disease management.

Pay

$28.51 - $34.68 per hour (base pay range).

Schedule

• Part-time, 32 hours per week.
• Evenings, 3:00 pm - 11:30 pm.
