About the role
Seeking dedicated Licensed Practical Nurses (LPNs) to provide personalized, one-on-one care in patients' homes for pediatric and adult populations.
Responsibilities
- Deliver skilled nursing care following physician orders and care plans
- Assess and monitor patient conditions, noting physical or emotional changes
- Collect, analyze, and document patient data for care coordination
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed
- Educate patients and families to promote safety and independence
Qualifications & Skills
- Active LPN license in Massachusetts
- Current BLS/CPR certification
- Current TB test or chest X-ray
- Strong clinical skills and attention to detail
- Compassionate, patient-focused approach
- One year of nursing experience preferred; new graduates encouraged
