Licensed Practical Nurse (LPN) - Home Health (Days) (PRN)

About the role

Identifies patient and family needs and provides clinical care in accordance with physicians’ orders and the hospice plan of care, following all policies and procedures of Hospice Care.

Responsibilities

• Deliver direct clinical care under physician direction and within the hospice plan of care.
• Assess and address patient and family needs in the hospice setting.

Requirements

• Completion of a technical or vocational school program.
• Up to two years of specialty training.
• One year of related experience.
• Working knowledge of standard practices and procedures in hospice or acute care.

Qualifications

• Minimum of one year experience as an LPN in an acute-care setting (preferred).
• Hospice experience preferred.
• Dependable automobile for patient visits.
• BLS certification required within 30 days of hire.
