About the role
We are seeking a dedicated LPN to join our team at Phoenix Home Health. You will be an integral part of the patient care team, working closely with physicians, therapists, and other healthcare professionals.
Responsibilities
- Affixes nursing care in accordance with physician’s plan of treatment and individual plan of care
- Observes, records, and reports client’s reaction to treatment and any changes in condition to appropriate personnel and/or physician
- Provides ongoing instruction of client or family in the plan of treatment
- Participates in education programs, quality assessment activities, nurse’s meetings, and other staff meetings
Requirements
- Missouri or Compact Registered Nursing or Licensed Practical Nursing License
- Strong organizational and self-management skills
- Strong and compassionate customer service skills
- Valid driver’s license
