Position Overview
Responsibilities
- Deliver exceptional, personalized home health care
- Conduct comprehensive patient assessments and create care plans
- Accurately document care activities and patient information
Qualifications & Skills
- Current, unencumbered LPN license
- Reliable transportation, valid driver’s license, and insurance
- CPR certification; additional certifications in home health or geriatrics preferred
- Skills in wound care, including wound vac management
