About the Role

As a Home Health LPN, you will provide skilled nursing care to patients in their homes while collaborating with the interdisciplinary care team to ensure high-quality, individualized care. You’ll work independently in the field—but never alone. Our Clinical Supervisors and operational staff are readily available to support you.

Responsibilities

  • Deliver skilled nursing care in accordance with the established plan of care
  • Monitor and assess patient conditions during home visits or shifts
  • Communicate changes in patient status to the Clinical Supervisor
  • Provide patient and caregiver education regarding medications, treatments, and disease management
  • Accurately document patient care and clinical observations
  • Coordinate care with other healthcare team members as appropriate
  • Maintain compliance with agency and state guidelines

Schedule

  • Day, evening, overnight shifts available
  • Weekday and weekend opportunities
  • Full-Time, Part-Time, PRN, Per Diem
  • Choose your own hours based on availability (3x12, 4x10, 5x8, and custom schedule options)

Pay

$30.00 - $36.00 per hour. Pay rate based on experience, case complexity, and assignment type. Opportunities for overtime and holiday scheduling are available.

Requirements

  • Active LPN license (Pennsylvania and/or Compact License)
  • Ability to work independently in a home setting
  • Strong communication and organizational skills
  • Reliable transportation
