About the Role
AdventHealth is seeking a Licensed Practical Nurse (LPN) to join our home health care team. This full-time position offers the opportunity to develop professional and clinical skills while providing direct patient care in a community-focused environment.
Responsibilities
- Develops professional and clinical skills through attendance at staff development meetings, pertinent educational activities, and professional affiliations
- Participates in the review process and the agency performance improvement program
- Provides patient education, medication administration, wound dressing changes, and ostomy care as directed by the RN and ordered by the physician
- Displays flexibility in accepting assignment changes and participates in weekend/after-hours coverage and call rotation
- Reports pertinent observations to the attending physician and RN, and implements changes in the patient care plan as directed
- Communicates changes to the appropriate members of the home health care team
- Demonstrates effective communication skills with all team members, patients, and families
- Maintains clinical record information in accordance with established standards and within established time frames
- Provides direct patient care responsibilities for assigned patients in an efficient, organized manner under the direction of the RN
- Performs treatments within their scope of training and administers medications to assigned patients
- Assesses patients during clinical check-in, documents pertinent medical information and vital signs, and assesses for signs of abuse or neglect using age-specific guidelines
- Administers treatments, medications, and follow-up assessments ordered by the provider, verifying as needed, and responds appropriately in clinical emergencies and triage situations
- Performs diagnostic and occupational medicine testing, including EKG, spirometry, audiometry, titmus, and pulse oximetry, and complete forensic testing collections in accordance with established guidelines
- Collects data specific to the age of the patient and contributes to their plan of care
- Receives, verifies, and implements physician orders, ensuring orders taken by the LPN are co-signed by an RN
- Other duties as assigned
Qualifications
- High School Grad or Equivalent (Required)
- Technical/Vocational School (Required) - Field of Study: LPN
- Licensed Practical Nurse (LPN) (Required)
- Basic Life Support - CPR Cert (BLS) (Required)
- IV Certified (IV) (Preferred)
- Driver's License (DL) (Required)
- Auto Insurance (AL) (Required)
- Background check meets organizational guidelines (Required)

- Basic computer skills required including but not limited to MS Office (Required); additional computer knowledge (Preferred)
- Venipuncture skills (Required)
- Must be proficient in the English language; additional languages (Preferred)
- Requires individuals to be able to make emergency and mature independent judgments and decisions (Required)
- Must have excellent communication skills, be flexible, and able to interact positively with associates (Required)
- Knowledge with a Point of Care software system for patient care documentation (Preferred)
Experience
- 1+ year of medical/surgical experience (Preferred)
Schedule
Full Time: Monday – Friday, 8am – 5pm, flexible, rotating weekends on-call
