Responsibilities

  • Develops professional and clinical skills through staff development meetings, educational activities, and professional affiliations.
  • Participates in the review process and agency performance improvement program.
  • Provides patient education, medication administration, wound dressing changes, and ostomy care as directed by the RN and ordered by the physician.
  • Demonstrates flexibility in accepting assignment changes and participates in weekend/after-hours coverage and call rotation.
  • Reports pertinent observations to the attending physician and RN, and implements changes in the patient care plan as directed.
  • Communicates changes to the appropriate members of the home health care team.
  • Maintains clinical record information in accordance with established standards and within established time frames.
  • Provides direct patient care responsibilities for assigned patients in an efficient, organized manner under the direction of the RN.
  • Performs treatments within their scope of training and administers medications to assigned patients.
  • Assesses patients during clinical check-in, documents pertinent medical information and vital signs, and assesses for signs of abuse or neglect using age-specific guidelines.
  • Administers treatments, medications, and follow-up assessments ordered by the provider, verifying as needed, and responds appropriately in clinical emergencies and triage situations.
  • Collects data specific to the age of the patient and contributes to their plan of care.
  • Receives, verifies, and implements physician orders, ensuring orders taken by the LPN are co-signed by an RN.
  • Other duties as assigned.

Qualifications

  • Demonstrates knowledge of practical nursing theory, modalities, treatments, and skills.
  • A high level of multitasking, organization, and proactive communication skills are necessary.
  • Problem-solving abilities.
  • Positive interpersonal communication skills.
  • Must possess sound judgment and critical thinking skills.
  • Team and customer service oriented.
  • Must be able to manage multiple details, tasks, and decision-making abilities under busy and stressful working conditions.
  • Basic computer skills required including but not limited to MS Office.
  • Additional computer knowledge preferred.
  • Venipuncture skills.
  • Must be proficient in the English language; additional languages preferred.
  • Background check meets organizational guidelines.
  • Requires individuals to be able to make emergency and mature independent judgments and decisions.
  • Must have excellent communication skills, be flexible, and able to interact positively with associates.
  • Knowledge with a Point of Care software system for patient care documentation preferred.

Skills

  • Licensed Practical Nurse (LPN).
  • Basic Life Support - CPR Cert (BLS).
  • IV Certified (IV).

Pay

$22.68 - $42.19 per hour.

Schedule

Full time, day shift Monday-Friday 8am-5:00pm - rotates call and weekends.
