About the Role
Under the supervision of an RN, the Licensed Vocational Nurse (LVN/LPN) provides direct patient care. The LVN/LPN observes and documents the patient's medical progress and nursing status changes under the direction of a registered nurse. This position provides direct patient care to pediatric, adolescent, adult, and geriatric patients in Clarke County, MS.
Responsibilities
- Performs technical nursing functions and direct patient care consistent with the established plan of care and under the supervision of an RN.
- Promotes and maintains optimal client care under the supervision of an RN.
- Maintains a safe patient environment.
- Identifies and communicates significant abnormalities in patients' data to case manager and other appropriate home health team members.
- Administers prescribed medications and treatments as permitted by law.
- Assesses all medicines a client may be taking to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, contraindicated medications, and reports any problems to the physician.
- Re-evaluates client’s nursing needs and reports findings to Case Manager.
- Counsels and instructs the client and family in meeting nursing and related needs.
- Prepares clinical and progress notes and submits all documentation in a timely manner in accordance with agency policy.
- Documentation on patients' chart correlates with nursing care plan.
- Communicates frequently and works closely with other home care team members.
- Assists in coordinating services provided to the client.
Requirements
- Graduate of an accredited practical or vocational nursing program.
- Currently Licensed Practical Nurse or Licensed Vocational Nurse in the state of agency operation.
- One year of LVN/LPN work experience.
- Ability to provide nursing visits according to the treatment as prescribed by the physician and provide and maintain paperwork to maintain accurate and complete medical records.
- Must possess and maintain valid CPR certification while employed in a clinical role.
- Must be a licensed driver who can travel to all business locations.
Pay
$48,000-54,000 annually. This position is paid on a per-point basis. The compensation reflected is an estimate of annual compensation.
Schedule
- Monday to Friday, 8:00 AM to 4:30 PM.
- No on-call requirement.
