Licensed Practical Nurse / LPN, Home Health

About the Role

Under the supervision of an RN, the Licensed Vocational Nurse (LVN/LPN) provides direct patient care. The LVN/LPN observes and documents the patient's medical progress and nursing status changes under the direction of a registered nurse. This position provides direct patient care to pediatric, adolescent, adult, and geriatric patients.

Responsibilities

• Performs technical nursing functions and direct patient care consistent with the established plan of care and under the supervision of an RN.
• Promotes and maintains optimal client care under the supervision of an RN.
• Maintains a safe patient environment.
• Identifies and communicates significant abnormalities in patients' data to case manager and other appropriate home health team members.
• Administers prescribed medications and treatments as permitted by law.
• Assesses all medicines a client may be taking to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, contraindicated medications, and reports any problems to the physician.
• Re-evaluates client’s nursing needs and reports findings to Case Manager.
• Counsels and instructs the client and family in meeting nursing and related needs.
• Prepares clinical and progress notes.
• Submits all documentation in a timely manner in accordance with agency policy.
• Ensures documentation on patients' charts correlates with the nursing care plan.
• Communicates frequently and works closely with other home care team members.
• Assists in coordinating services provided to the client.

Requirements

• Graduate of an accredited practical or vocational nursing program.
• Currently Licensed Practical Nurse or Licensed Vocational Nurse in the state of agency operation.
• One year of LVN/LPN work experience.
• Ability to provide nursing visits according to the treatment as prescribed by the physician and provide and maintain paperwork to maintain accurate and complete medical records.
• Must possess and maintain valid CPR certification while employed in a clinical role.
• Must be a licensed driver who can travel to all business locations.

Pay

$48,000-$54,000 annually. This position is paid on a per-point basis. The compensation reflected is an estimate of annual compensation.

Schedule

Monday to Friday, 8:00 AM - 4:30 PM. Volunteer for SNP (Special Needs Plan) on-call opportunities.
