About the Role

Under the supervision of an RN, the Licensed Vocational Nurse (LVN/LPN) provides direct patient care to pediatric, adolescent, adult, and geriatric patients. This position observes and documents the patient's medical progress and nursing status changes under the direction of a registered nurse.

Responsibilities

  • Performs technical nursing functions and direct patient care consistent with the established plan of care and under the supervision of an RN.
  • Promotes and maintains optimal client care under the supervision of an RN.
  • Maintains a safe patient environment.
  • Identifies and communicates significant abnormalities in patients' data to case manager and other appropriate home health team members.
  • Administers prescribed medications and treatments as permitted by law.
  • Assesses all medicines a client may be taking to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, contraindicated medications, and reports any problems to the physician.
  • Re-evaluates client’s nursing needs and reports findings to Case Manager.
  • Counsels and instructs the client and family in meeting nursing and related needs.
  • Prepares clinical and progress notes and submits all documentation in a timely manner in accordance with agency policy.
  • Ensures documentation on patients' charts correlates with the nursing care plan.
  • Communicates frequently and works closely with other home care team members.
  • Assists in coordinating services provided to the client.

Requirements

  • Graduate of an accredited practical or vocational nursing program.
  • Currently licensed as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of agency operation.
  • One year of LVN/LPN work experience.
  • Ability to provide nursing visits according to the treatment as prescribed by the physician and maintain accurate and complete medical records.
  • Must possess and maintain valid CPR certification while employed in a clinical role.
  • Must be a licensed driver who can travel to all business locations.

Pay

$49,000–$54,000 annually. This position is paid on a per-point basis. The compensation reflected is an estimate of annual compensation based on years of experience.

Schedule

  • Monday–Friday, 8:00 AM–4:30 PM.
  • Occasional on-call assistance may be required to assist with standard visits if primary and backup on-call staff are too full on Saturdays.
