Key Responsibilities

• Provide skilled nursing care, including medication administration, wound care, catheter care, and IV therapy
• Conduct patient assessments and contribute to individualized care planning
• Monitor patient conditions and report changes appropriately
• Educate patients and family members regarding treatment plans and health management
• Maintain accurate and timely clinical documentation in compliance with agency and regulatory standards
• Ensure patient safety while upholding the highest standards of nursing practice
• Collaborate with interdisciplinary healthcare teams and community providers

Qualifications

• Active New York State LPN License
• Minimum of 1–2 years of nursing experience preferred (Home Care, Pediatrics, or Geriatrics)
• Strong clinical judgment and ability to work independently
• Current CPR/BLS Certification
• Experience with Electronic Health Records (EHR) preferred
• Excellent communication, organizational, and interpersonal skills
• Reliable transportation and/or valid driver's license preferred
