About the role
The Licensed Practical Nurse (LPN) plays a crucial role in our ClearSky Rehabilitation Hospital, supporting patients with their care needs under the guidance of Registered Nurses. This position is integral to our mission of providing high-quality, compassionate care.
Responsibilities
- Provides skilled nursing care to patients as ordered by the physician and under the supervision of a Registered Nurse.
- Implements orders for medications and treatments as ordered by the physician and in accordance with the nursing care plan.
- Communicates with the Registered Nurse to develop and update individualized plans of care for each patient.
- Evaluates the effectiveness of interventions and seeks guidance from the RN when needed.
- Reports patient care to the supervisor and identifies patient problems and potential risks.
- Assists in identifying patient learning needs and reinforcing self-care techniques.
- Accurately assigns charges to individual patients using the hospital’s established system.
- Maintains a safe environment for patients, visitors, and staff, assessing risks and implementing appropriate precautions.
- Complies with safety and infection prevention standards.
Requirements
- Current state license as a Licensed Practical Nurse.
- Current BLS certification.
- Two years of experience in inpatient hospital settings, with inpatient rehabilitation experience preferred.
- Knowledge of and adherence to current nursing theory and practice, as well as infection prevention standards.
- Understanding of clinical operations and procedures, and the ability to apply policies, procedures, and guidelines.
- Basic computer skills including data entry, word processing, email, and records management.
- Effective organizational and time management skills.
- Strong written and verbal communication skills.
- Ability to prioritize tasks and meet deadlines.
- Proper levels of confidentiality maintained.
Qualifications
- High school diploma or equivalent.
- Graduation from an accredited LPN program.
