Licensed Practical Nurse, LPN Full-Time (Evergreen)

About the role

The Licensed Practical Nurse (LPN) plays a crucial role in our ClearSky Rehabilitation Hospital, supporting patients with their care needs under the guidance of Registered Nurses. This position is integral to our mission of providing high-quality, compassionate care.

Responsibilities

• Provides skilled nursing care to patients as ordered by the physician and under the supervision of a Registered Nurse.
• Implements orders for medications and treatments as ordered by the physician and in accordance with the nursing care plan.
• Communicates with the Registered Nurse to develop and update individualized plans of care for each patient.
• Evaluates the effectiveness of interventions and seeks guidance from the RN when needed.
• Reports patient care to the supervisor and identifies patient problems and potential risks.
• Assists in identifying patient learning needs and reinforcing self-care techniques.
• Accurately assigns charges to individual patients using the hospital’s established system.
• Maintains a safe environment for patients, visitors, and staff, assessing risks and implementing appropriate precautions.
• Complies with safety and infection prevention standards.

Requirements

• Current state license as a Licensed Practical Nurse.
• Current BLS certification.
• Two years of experience in inpatient hospital settings, with inpatient rehabilitation experience preferred.
• Knowledge of and adherence to current nursing theory and practice, as well as infection prevention standards.
• Understanding of clinical operations and procedures, and the ability to apply policies, procedures, and guidelines.
• Basic computer skills including data entry, word processing, email, and records management.
• Effective organizational and time management skills.
• Strong written and verbal communication skills.
• Ability to prioritize tasks and meet deadlines.
• Proper levels of confidentiality maintained.

Qualifications

• High school diploma or equivalent.
• Graduation from an accredited LPN program.
