Lee County Care & Rehabilitation Center is a 109-bed skilled nursing facility located along the middle fork of the Kentucky River in Beattyville, Kentucky. The facility offers short-term rehabilitation services as well as long-term care, including physical, occupational, and speech therapies, and ongoing restorative therapy to maintain residents' highest level of mobility and independence. About the role

As a Licensed Practical Nurse (LPN), you will provide direct nursing care to residents, fostering strong connections and making a meaningful impact. This role offers opportunities for career growth through meaningful work, professional development, and a supportive team environment.

Responsibilities

  • Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
  • Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
  • Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
  • Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
  • Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
  • Complete accurate and timely clinical documentation per facility policy and procedure.
  • Coordinate resident admissions, transfers, and discharges according to established procedures.
  • Participate in quality assurance activities, staff education, and regulatory surveys as needed.
  • Perform other duties as assigned by the Director of Nursing or designee.

Requirements

  • Current, active LPN/LVN license in the state of practice.
  • CPR/BLS certification preferred (American Heart Association or American Red Cross).
  • Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
  • Experience in a supervisory or charge nurse role preferred.
  • Strong clinical observation, medication administration, and documentation skills.
  • Knowledge of Federal and State long-term care regulations.
  • Effective verbal and written communication skills.
  • Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
  • Familiarity with electronic health records (EHR) systems.
  • Proficiency with Microsoft Office applications.

Pay

Up to USD $28.97 per hour, with a $2,500 sign-on bonus for full-time nurses.
