About the role

The Licensed Practical Nurse (LPN) plays a crucial role in our ClearSky Rehabilitation Hospital, supporting patients in their recovery journey. Our mission is to provide high-quality care that transforms lives, and we are seeking dedicated professionals to join our team.

Responsibilities

  • Provides skilled nursing care to patients as ordered by the physician and under the supervision of a Registered Nurse.
  • Implements orders for medications and treatments as ordered by the physician and in accordance with the nursing care plan.
  • Participates in the development and evaluation of the effectiveness of interventions in relation to patient responses.
  • Reports to the supervisor on patient care rendered, problems concerning patients' needs, and resolution to those needs.
  • Aids in identifying patient problems and potential risks and contributes to the development of interventions.
  • Assists in identifying patient's learning needs and reinforces self-care and other techniques.
  • Appropriately designates applicable charges to individual patients using the hospital's established system.
  • May be required to work during inclement weather and other staffing emergencies.
  • Ensures an environment conducive to safety for patients, visitors, and staff.
  • Assesses risks for safety and implements appropriate precautions.
  • Complies with appropriate and approved safety and infection prevention standards.

Requirements

  • Minimum Education & Experience: Two years’ experience in inpatient hospital setting, inpatient rehabilitation experience preferred.
  • Required Licenses, Certifications, And/or Documentation: Current state license as a Licensed Practical Nurse required. Current BLS certification required.

Qualifications

  • Knowledge of and adherence to current nursing theory and practice and infection prevention standards of practice.
  • Knowledge of accreditation standards to ensure adherence to all standards set forth by state and accrediting agencies of TJC and CMS.
  • Knowledge of clinical operations and procedures, and utilizes proper policies, procedures, and guidelines in caring for patients.
