Role & Responsibilities
- Provide skilled nursing care to patients across various acuity levels, including G-tube, Trach, and Vent support.
- Assist with urgent cases, ensuring clinical outcomes are achieved with empathy and professionalism.
- Utilize electronic documentation tools in patient homes for efficient record-keeping.
- Collaborate with a team of clinicians and support staff to deliver outstanding care.
Qualifications & Skills
- Active, unencumbered LPN license.
- Current CPR certification; assistance available for obtaining if needed.
- TB skin test within the last 12 months.
- Reliable transportation and at least six months of hands-on nursing experience preferred.
