About the role

RiseWell Community Services is seeking a dedicated Licensed Practical Nurse (LPN) to join our Assertive Community Treatment (ACT) Team in West Babylon, NY. This part-time position offers a unique opportunity to make a meaningful impact in the lives of individuals living with serious mental illness by providing community-based, recovery-oriented support.

Job Function

Provide management and administration of medications, in conjunction with the psychiatrist, and registered nurse as a member of a mobile, multidisciplinary treatment team for persons with severe and persistent mental illness.

Qualifications

  • Currently licensed as a Licensed Practical Nurse by the New York State Education Department.
  • Valid and clean NY State driver license.
  • Minimum of 1 year of psychiatric experience in a treatment or rehabilitation setting.
  • Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

Essential Functions

  • Conducts routine mental status exams.
  • Affords physical health needs.
  • Makes appropriate referrals to community physicians and community based services.
  • Manages and administers medications and IM injections in conjunction with psychiatrist.
  • Participates on team that delivers an integrated array of treatment, rehabilitation and support services, customized to each person.
  • Shares in the joint responsibility for integrated plan.
  • Participates in team interactions producing regular ongoing transfer of information and skills.
  • Provides the majority of services in community.
  • Participates in rotating 24-hour crisis coverage and response.
  • Complies with agency Compliance Plan and False Claims Policy by preventing, detecting and reporting any potential healthcare abuse and fraud.
  • Reports any potential non-compliance and complies with all other federal, state and local regulations applicable to this job description.
  • Utilizes all agency based IT applications pertaining to this job description in a proficient manner.
  • Remains current on changing developments in individual field of expertise.

Competencies

  • Knowledge of nursing assessment methods.
  • Ability to work within team setting.
  • Skilled in counseling techniques through education, or training, or work experience.
  • Ability to work as a generalist.
  • Maintains licensed as a practical nurse in New York State.
