- Pay rate: $34.48 – $37.93 per hour

- Shifts available: 7:30 am – 3:30 pm, 3:30 pm – 11:30 pm, 11:30 pm – 7:30 am
- Full‑time, part‑time, and PRN positions

Responsibilities
- Admissions, discharge, and transfers of residents
- Medication requisitions with pharmacy and physicians
- Medication administration
- Supervising Certified Nursing Assistants (CNA) as required
- Following infection control, emergency, and other center policies
- Collaborating with the resident’s interdisciplinary team to achieve optimal outcomes
- Identifying and reporting abnormal changes in residents’ condition per policy
- Delivering nursing care to patients/residents requiring long‑term or rehabilitative care
- Collecting patient/resident data, making observations, and reporting pertinent information
- Implementing and evaluating patient/resident plan of care according to state regulations
- Directing and supervising care given by other nursing personnel in selected situations
- Maintaining knowledge of documentation requirements
- Maintaining knowledge of equipment set‑up, maintenance, and use (e.g., monitors, infusion devices, drain devices)
- Upholding confidentiality and patient/resident rights regarding all information
- Working under direct supervision in accordance with the state Nurse Practice Act, facility policies, and nursing judgment
Qualifications
- Licensed Practical Nurse (LPN) licensed by the State of New York
- CPR certification required
- Two years prior nursing experience preferred
