Premier HealthCare, a member of the YAI Network, seeks a Licensed Practical Nurse (LPN) for our multi-specialty clinic serving individuals of all ages with intellectual and developmental disabilities (IDD).

Responsibilities

  • Report to the nurse manager or supervisor; attend weekly supervision meetings.
  • Oversee day-to-day operations of the nursing station, ensuring workflows and policies are followed.
  • Assume duties of the nurse supervisor when needed; provide coverage for PCMH and other grants.
  • Supervise nursing staff, including Medical Assistants and medical secretaries.
  • Use proper PHC greeting according to policy script 100% of the time.
  • Assign LPNs or Medical Assistants to assist each provider with daily workflow.
  • Document all activities in the electronic medical record (EMR) system.
  • Function as a Patient-Centered Medical Home (PCMH) care team member.
  • Coordinate and manage high-risk populations; lead and participate in daily huddles.
  • Prepare for daily huddles by reviewing high-risk groups, pending referrals, labs, testing, immunizations, and visit reasons.
  • Participate in monthly care team meetings and provide patient updates.
  • Ensure exam rooms are prepared, computers are operational, and report any issues.
  • Escort patients to and from exam rooms; verify patient demographics and assigned PCP in the EHR.
  • Obtain and document vital signs, height, weight, BMI, and all pertinent patient information in the EHR with 100% accuracy.
  • Complete the PCMH tab in the EHR; provide education sheets to high-risk patients and reinforce provider-set goals.
  • Obtain and update medication lists; notify providers of any changes and complete medication reconciliation.
  • Collect forms or documents for provider review and sign-off.
  • Prepare medical records for clinicians, including lab reports, diagnostic results, and progress notes.
  • Obtain pending imaging or testing results from patients or caregivers.
  • Perform phlebotomy, EKGs, and other office procedures as ordered by clinicians.
  • Administer vaccinations, medication standing orders (MSO), and PPD plantings within scope of practice.
  • Complete intake packets and updates; enter information into the EHR.
  • Assist with patient examinations and therapeutic interventions to ensure safety.
  • Assist with check-out processes: print patient plans, referrals, or prescriptions; schedule return visits.
  • Enter prescription requests, form requests, and other documentation into designated databases.
  • Obtain prior authorizations for non-covered medications or feeding supplies.
  • Notify patients of lab results as directed by providers and document in the EHR.
  • Maintain cleanliness and organization of treatment rooms and practice areas.
  • Dispose of biohazardous waste, including sharps, and replace disposal units as needed.
  • Promote continuity of care as a liaison between PHC, patients, families, caregivers, and other agencies.
  • Participate in telephone triage to prioritize requests and ensure necessary documentation.
  • Document hospital/ER follow-ups in the EHR; obtain and review discharge summaries.
  • Participate in quality improvement activities as directed.
  • Resupply and inventory medical supplies as assigned.
  • Comply with OSHA/CLIA regulations and report safety/health hazards.
  • Assist with emergencies as directed by the charge nurse or clinician.
  • Comply with NYSDOH regulations and PHC policies and procedures.
  • Check refrigerator/freezer temperatures and emergency equipment daily.
  • Ensure emergency carts and medications are up to date; check monthly.
  • Monitor vaccine inventory and expiration dates weekly; ensure proper labeling and updated vaccine information statements.
  • Maintain competencies through training, continuing education, and reference materials.
  • Endorse and receive information before/after breaks; review and respond to emails within the same day.
  • Complete all assigned tasks daily; perform other duties as designated by supervisors.

Requirements

  • Licensed Practical Nurse (LPN) license.
  • Current CPR certification.
  • Ability to operate necessary computer equipment and software programs.
  • Excellent written and verbal communication skills.
  • Self-motivated with strong interpersonal and customer service skills.
  • Well-organized, detail-oriented, and able to prioritize tasks in a fast-paced environment.

Preferred Qualifications

  • Experience with Electronic Health Record (EHR) and scheduling systems.
  • Experience in an outpatient medical facility.
  • Experience with individuals with intellectual and/or developmental disabilities.
