Licensed Practical Nurse (LPN)

Responsibilities

• Conduct thorough assessments to ensure personalized, comprehensive care.
• Guide and educate patients and their families on health maintenance and disease prevention.
• Provide gentle assistance with Activities of Daily Living (ADLs) as needed, always with dignity and respect.
• Coordinate schedules directly with patients, aligning with their plan of care, approved hours, and personal preferences.
• Monitor and promptly report any changes in patient symptoms or behavior.
• Administer medications and treatments with precision and care, as prescribed by their treating physician.
• Communicate seamlessly with Case Managers and collaborating physicians or specialists, ensuring a unified approach to patient care.
• Maintain accurate and heartfelt patient logging in our EMR system.
• Offer valuable advice and heartfelt emotional support to patients and their family members.

Qualifications

• A valid and active LPN License in the state of practice.
• A willingness to travel, bringing care directly to patients' homes.
• Graduation from an accredited school of nursing.
• A current BLS Certification.
• The ability to pass a state & federal background check.
• Malpractice Insurance.
• Strong problem-solving and critical thinking skills, coupled with a warm, empathetic approach.

Pay

Pay Range: USD $28.00 - USD $32.00 /Hr.
