About the role
Join our dedicated team in Rockingham to support the community's health as an LPN! This dual role balances specialized Primary Care Access Plan (PCAP) Specialist with vital clinic backup services, ensuring seamless patient care, health screenings, and medical administration under the supervision of a Public Health Nurse.
Responsibilities
- PCAP Duties: As the PCAP Care Management Specialist, work interdependently with other medical professionals to provide quality health care to patients. Act as a recruiter for patients who are uninsured or underinsured to provide medical access at a low cost. Serve as a care manager to help patients maintain good health practices.
- Clinic Back-Up: Provide hands-on clinical support by taking vital signs, administering medications and immunizations, and preparing patients for examinations.
- Documentation: Maintain accurate, confidential patient records in accordance with North Carolina state and federal public health regulations.
Requirements
- Graduation from a state-accredited practical nursing program.
- Active, unencumbered LPN license to practice in North Carolina.
- Current CPR/BLS certification.
- Minimum of 1 year of clinical nursing experience, preferably in primary care or community/public health setting.
- Reliable transportation for occasional local community outreach.
