About the role

We are hiring for a Licensed Practical Nurse (LPN) at RHA Health Services. This is a Regular position.

Responsibilities

  • Provides prescribed medical treatment and personal care and services to persons with disabilities in residential homes and/or vocational centers at a single business location.
  • Relies on knowledge, experience, and judgment to accomplish tasks in combination with clinical supervision from the registered nurse and direction from the Registered Nurse and/or Administrator.
  • Requires graduation from an accredited licensed nurse program and current LPN license with no restrictions to practice nursing.
  • Reports to: Regional Nursing Director
  • Coordinates preventative and routine medical care for people supported.
  • Documents nursing care provided.
  • Assists physicians with on-site medical clinics where applicable.
  • Provides health related trainings to unlicensed personnel according to state regulations.
  • Conducts medication pass observations in the home to ensure staff competency according to state regulations.
  • Conducts medication closest assessments.
  • Conducts quality assurance checks and observations to ensure unlicensed personnel are performing health related tasks as assigned according to state regulations.
  • Observes for and reports any abnormal health related findings to the physician.
  • Communicates with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team.
  • Advocates for the people supported and assists with developing community supports to promote optimal health for the people supported.

Qualifications

  • Graduation from an accredited licensed nurse program.
  • Current LPN license with no restrictions to practice nursing.
  • Current CPR Certification or eligibility to certify for CPR.
  • Possess credentials required by Federal or State law.
  • Able to communicate effectively with Interdisciplinary Team Members, as well as the people supported and their families.
  • Able to work independently, be organized, and possess the ability to prioritize work tasks.
  • Able to regularly lift and carry up to 20 lbs. and occasionally pull at least 40 lbs.
  • Able to squat, kneel, crawl, crouch, climb, and stoop as part of regular job duties.
  • Able to use hands and fingers to handle or operate objects, tools, or controls.
  • Able to stand and walk for extended periods.
  • Able to demonstrate proficiency in CPR from floor level, which requires working on hands and knees, bending, standing, and lifting.
  • Has vision requirements including close, distance, and peripheral vision.
  • Can talk and hear.
  • Exception – Deaf and Hard of Hearing Programs: Fluency in American Sign Language (ASL).
  • At least 18 years of age.
