Licensed Practical Nurse (LPN)
About the role
The Licensed Practical Nurse (LPN) supports the coordination and delivery of healthcare services for individuals across multiple programs. This role works closely with the Registered Nurse and interdisciplinary teams to ensure quality care, regulatory compliance, and effective staff training.
Responsibilities
- Monitor health conditions and report changes to the RN
- Care coordination with physicians, pharmacy, therapy, and other providers
- Administer, track, and oversee medication processes and documentation
- Aid in developing and updating care plans and health assessments
- Train and support staff on medication administration, medical procedures, and infection control
- Ensure accurate, timely documentation and compliance with regulations
- Participate in interdisciplinary team meetings and provide health updates
- Conduct audits, track appointments, and monitor follow-up care
- Support infection control practices and program quality assurance
- Provide general nursing care to people receiving services
- Participate in on-call rotation and travel for trainings/meetings as needed
Requirements
- A valid, unrestricted driver’s license
- Current Minnesota LPN license and completion of an accredited practical nursing program
- At least 2 years of nursing experience preferred, especially working with individuals with varying abilities
- Willingness to complete and pass a background check and MVR screening after offer

Pay
$26.00-$27.00 per hour
Schedule
Monday through Friday with flexible hours between 8:00am-5:00pm. 40 hours per week.
