Responsibilities

• Provides high quality clinical services within the scope of practice, as defined by state laws governing nursing practice, in accordance with the plan of care and in coordination with other health‑care team members, following all applicable infection control procedures.
• Documents patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record using the agency's electronic visit verification program (as applicable).
• Actively participates in the agency's care coordination process including timely reporting of patients' needs and changes in condition, attending patient case conferences, and communicating effectively with the patient, family, physician, other clinicians, agency management staff, case coordinator, and community case managers (as applicable).
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Requirements

• Current and unrestricted LPN licensure in the state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or reliable public transportation.

Qualifications

• Current CPR certification or ability to complete within 90 days of hire.
• Home care experience.
• Ability to manage multiple tasks simultaneously.
• Able to work independently.
• Good communication, writing, and organizational skills.
• Ability to work flexible hours as required to meet identified client needs.

Pay

Annual total cash target pay: $46,564 – $69,846.

Per visit point rate: $26.86 – $40.30 per visit point.

Hourly rate: $22.39 – $33.58 per hour.

Total cash compensation includes earnings from per‑visit point pay and hourly pay, based on factors such as local labor markets, education, work experience, and may increase with productivity and performance. The role assumes full‑time employment (40 weekly hours) at full productivity for the annual range.

Schedule

Full‑time schedule (approximately 40 hours per week).
