Job Summary

As a Licensed Practical Nurse, you’ll provide direct and indirect nursing care that supports each participant’s individualized plan. You’ll work closely with the Clinic Nurse Manager, RN, and the care team to deliver safe, accurate, and compassionate care. This position is ideal for someone who is patient, dependable, and comfortable working within established routines.

Duties/Responsibilities

  • Provide direct and indirect nursing care to participants.
  • Collect participant data and complete required documentation accurately and according to standards.
  • Aid in planning, implementing, and evaluating individualized plans of care.
  • Support coordination of services by communicating participant needs and changes to the care team.
  • Participate in the development of the IDT Plan of Care and ensure proper follow-through.
  • Provide treatments, procedures, and care as prescribed and according to practice standards.
  • Educate participants and families on health needs and care instructions.
  • Document all care in the electronic medical record system.
  • Support quality improvement activities.
  • Follow established guidelines, policies, and participant safety protocols.
  • Other duties as assigned.

Requirements

  • Graduate of an accredited nursing school and have a current state LPN license.
  • One year of experience working with the frail or elderly population, or willing to undergo training to supplement this requirement, which may include overnight travel.
  • Valid State driver’s license with a clean driving record.
  • Successful completion of background check, physical, drug screening, TB Screening.
