Licensed Practical Nurse LPN

About the Role

Under the supervision of a Registered Nurse, you will provide skilled nursing services as ordered on the plan of care and according to policy/procedures and state practice acts. This includes instructing patients and caregivers in patient care and disease management, documenting comprehension and compliance, observing patient responses, and recommending revisions to the plan of care. You will also perform other duties as assigned by the RN or Clinical Manager.

Responsibilities

• Provide skilled nursing services according to the established plan of care, policies/procedures, and standards of care.
• Instruct the patient and caregiver in various aspects of patient care and disease management.
• Document the patient/caregiver comprehension of and compliance with teaching.
• Observe patient response to care and teaching; make recommendations regarding revision of the plan of care to the Registered Nurse.
• Perform other duties as assigned by the RN or Clinical Manager.

Requirements

• Unencumbered, current Licensed Practical Nurse license in the state where the HomeCare agency is located or compact state if applicable.
• Graduate of a state-approved LPN program.
• Minimum one (1) year of clinical experience as an LPN (individuals with less than one year may be considered but will receive mentorship and additional oversight).
• Experience in home health care is desirable.
