Pay

$30.00 - $32.00 / hour Depending On Experience

About the Role

Serves patients throughout the Southeast region, providing hospice care with grace. Supports team members, patients, and families with compassion, accountability, respect, integrity, and service.

Responsibilities

  • Provides technical care to patients, receives assignments and reports
  • Performs prescribed medication under supervision of RNO
  • Observes, records, and reports all observations of the patient’s mental and physical care
  • Notifies Patient Care Coordinator and/or physician of any changes in patient’s condition; documents changes in the clinical record
  • Maintains regular contact with Patient Care Coordinator for discussion of nursing care, policies, and future planning
  • Participates as an IDT team member in formulating the interdisciplinary care plan
  • Aids home health aides in their duties
  • Assists physicians or RNs in performing specialized procedures
  • Accurately utilizes automated devices to document in the electronic medical record
  • Participates in on-call schedule

Requirements

  • Licensed in the agency state of operation
  • At least one (1) year of nursing experience
  • Knowledge of technical skills required in nursing procedures
  • Understanding of hospice care and role as part of an interdisciplinary care team

  • Licensed Practical Nurse (LPN)
  • One (1) year of nursing experience
  • Complete knowledge of technical skills required in nursing procedures
  • Understanding of hospice care and role as part of an interdisciplinary care team
